In South Carolina, there are three types of exemptions from immunizations, in which notarization may be required by a scholastic institution: Medical, Religious, and Special.
Medical Exemption
A South Carolina Certificate of Medical Exemption may be granted to a child who has a chronic or temporary medical condition that contraindicates a vaccine. This certificate must be completed and signed by a licensed physician and submitted to the school or childcare facility.
Religious Exemption
If a parent/guardian has religious beliefs that prevent a child from receiving immunizations, a South Carolina Certificate of Religious Exemption can be obtained. To get this certificate, the parent/guardian must go to the local health department and sign the appropriate section of the form affirming that one or more immunizations conflict with their religious beliefs. This signed document must then be submitted to the school or childcare facility.
Special Exemption
This is for children in foster care or under the custody of the Department of Social Services. The Department of Social Services must submit a written statement to the school or childcare facility.
